For many people back pain links with inflammation, but the link is not the whole story. Stress associates with chronic low back pain in population research, although this does not prove cause and effect.

Cortisol is part of the stress response. Chronic pain and chronic stress often show cortisol dysregulation over time, with patterns varying by person and measurement method. The practical takeaway is simple: repeated high stress days often pair with worse pain, worse sleep, and more cravings for high sugar foods, which then raises inflammatory load.

Anti inflammatory eating, what the evidence supports

There is growing nutrition research specific to low back pain. A narrative review summarises links between diet quality, inflammation, and low back pain, including the role of obesity and nutrient gaps.

Studies using diet inflammation scoring (Dietary Inflammatory Index) suggest more pro inflammatory eating patterns link with worse musculoskeletal outcomes in some samples, and diet quality differences show up between chronic low back pain groups and controls in some studies.

    Pick two:
    Same wake time daily
    Morning daylight for 10 minutes
    Caffeine cut off mid afternoon
    Screens off 60 minutes before bed
    Cool, dark room
    Short wind down routine, shower, stretch, breathing
  2. Resistance training baseline
    Aim for 2 to 3 sessions per week:
    Hip hinge pattern, light RDL or kettlebell deadlift
    Squat pattern, sit to stand or goblet squat
    Pull, row variation
    Carry, farmer carry
    Trunk, side plank or dead bug

Progress slowly for 12 weeks. Consistency beats intensity.

  1. Reduce cortisol spikes through rhythm
    Do:
    Protein at breakfast
    A short walk after meals
    Breathing breaks during high stress windows
    Earlier bedtime on stressful days
    Strength training rather than long punishing cardio if you feel wired and tired
